   I have spoken with my doctor, who conforms that I am an extremely vulnerable person and should receive help. He did not know of any form that he should be filling in.ju e Irrelevant s

   After trying to answering tou questions many times I am confused. I confirm that I some to my doctor who confirmed that he confirmed that I was an extremely vulnerable person with life long Asthma/COPD and as such should be registered
